STREET TO HOME 2.0

In 2023, FCI launched Street to Home 2.0 with coalition partners (Akido & HOPICS) to accelerate housing placement and improve housing retention. Together coalition partners designed a new approach that eliminates the numerous case manager handoffs typically experienced by unhoused individuals and, rather, assigns a single case manager who can follow an individual from street to home. Moreover, this model provides the critical medical care unhoused individuals need in-house and on-site, including mental health services, addiction medicine, and chronic disease management. Ultimately, this initiative aims to achieve faster housing placements and better retention rates.

Impact

In the past year, we have successfully secured funding and implemented this care model in Kern and Los Angeles Counties. Preliminary data shows that we can go from the initial visit to securing a housing voucher in as little as four weeks. This initiative also demonstrates how directing homeless services funding can increase efficiency and effectiveness.
